If you’ve been told that weekly therapy isn’t enough, or you’re stepping down from a higher level of care, you may be hearing two terms a lot: PHP and IOP. Both are intensive, structured outpatient programs. Both use evidence-based group therapy. But they’re not the same, and choosing the right one matters.
Starting at the wrong level can mean either too little support to make real progress or more structure than your current situation requires. Understanding the difference helps you — and the people who care about you — make a more confident, informed decision.
What Is a Partial Hospitalization Program (PHP)?
A Partial Hospitalization Program is the most intensive level of outpatient care available. At Connections Wellness Group, PHP involves attending structured programming for four to six hours per day, five days per week. You go home each evening; this is not inpatient treatment.
A PHP mental health program is designed for people whose symptoms are acute or significantly impacting daily functioning; those who need daily clinical support but don’t require overnight hospitalization. It’s also a common mental health step-down program for individuals transitioning out of inpatient or residential care.
What Is an Intensive Outpatient Program (IOP)?
An Intensive Outpatient Program offers the same evidence-based group therapy modalities — CBT, DBT, and ACT — on a reduced schedule of roughly nine to twelve hours per week. IOP is built to fit into real life.
It’s a strong fit for people whose symptoms are moderate or stabilizing, and who can manage daily responsibilities, such as work, school, or family, while still getting structured, clinically rigorous support. Many people enter IOP directly; others move into it from PHP as they progress.
What Conditions Are Treated at Both Levels?
Both PHP and IOP address a wide range of mental health conditions, including:
- Depression and mood disorders
- Anxiety and panic disorders
- PTSD, trauma, and grief
- OCD and personality disorders
- Psychosis and neurodivergence
- Co-occurring conditions (Dual Diagnosis)
- LGBTQIA+ mental health needs
- Behavioral and relationship challenges
Key Differences Between PHP and IOP
| PHP | IOP | |
| Hours per Week | ~25–30 hours | ~9–12 hours |
| Acuity Level | Acute or unstable symptoms requiring daily clinical structure — a step below inpatient, a step above standard outpatient | Moderate symptoms or a stabilizing clinical picture; appropriate after PHP or as an entry-level intensive option |
| Designed For | People who need daily support but do not require overnight or residential care | People who can maintain some daily responsibilities (work, school, family) alongside structured treatment |
| Program Structure | Group therapy + individual psychiatric support + medication management — multiple sessions per day | Group therapy sessions using the same evidence-based modalities (CBT, DBT, ACT) — fewer hours, same clinical rigor |
| Life Integration | Requires significant daily availability — typically not compatible with full-time work or school during active enrollment | Designed to fit into daily life — many clients maintain part-time or full-time commitments alongside IOP |
| Virtual Option | Virtual PHP programming for Texas residents statewide | Virtual IOP programming for Texas residents statewide |
| Insurance Coverage | Covered by many plans as medically necessary behavioral health care — verify benefits before first appointment | Covered by many plans as medically necessary behavioral health care — verify benefits before first appointment |
